BILLING AND FEES

Please be aware of the following billing practices:

NOTE: For our self-pay patients, we can provide an itemized receipt or super-bill that may allow you to get some reimbursement from your insurance company if you have out-of-network benefits.

We do not offer a sliding scale or discounted rates.

We are close to full on our patient list, and as we want to spend time with all of our patients please understand that appointment cancellation fees and rescheduling 1-2 weeks out are the only way to continue keeping appointment times as scheduled. Thanks.

CLINIC TIME BETWEEN APPOINTMENTS

Clinical services provided between appointments (for example phone calls outside of visits, letters or forms, and transfer of records) are billed at the rate of $50 per 10-minute increment.

We do not complete forms/letters for:

Short term disability
Long term disability
VA disability/SSI disability
FMLA
Time off work
Jury Excuses
Probation Officer Letters
Any other letters for legal involvement that will take time away from our provider's ability to see other patients.

Please keep in mind, that any time taken away from our day is taken away from another patient's time slot.
